Quasi-Government System: A Proposal for Arranging Helathcare Service in Indonesia

Three basic types of healthcare system are widely use around the world : tax funded national health care also known as single payer plan, private insurance and compulsory social insurance. even though many countries have achieved good health care system, the challenges of intensively discussing to find out a high quality healthcare system at low costs is still attracting experts, politicians and insurance practitioners. Germany and Japan's system are frequently pointed out as the good examples for high quality healthcare with low costs. The systems have been providing all people, regardless their income, high quality healthcare service while constraining costs. The system in both countries are arranged by quasi-government organizations, non-profit organizations with exclusive right to collect premiums and contract providers. Every person with ceiling income has to contributes to the system.
Indonesia is a developing country struggling to build a good healthcare system. Indonesia considers two aspects, healthcare delivery service and healthcare financing in building a good health care system. The system should assure members from financial lost while getting sick, bargaining position with providers, high quality and affordable on the basis of social principles, egalitarian and comprehensiveness. The membership should be compulsory in which people with ceiling income have to contribute to the system. The system also should include health prevention and promotion. By combining other country's  experiences and Indonesian's existing conditions, a compulsory social insurance, which is arranged  by quasi-government organizations, sounds appropriate for Indonesia. There are several determinant factors supporting implementation of the systems such as social concepts, economic cost constrain, ideology, existing arrangers, undesired impacts of decentralization, new tendency about rights health and high poverty rate. In term to build the system, an implementation plan need to be arranged
